The Commission has modernised and simplified the Personal Questionnaire (Form PQ) so that it is easier to complete and has included changes to take account of the coming into force of the Rehabilitation of Offenders legislation. The guidance for completing the form, which was previously part of the form, has been updated and moved into a separate document. The Personal Declaration (Form PD) has also been amended so that a Form PQ can be updated by Form PD for a period of five years rather than two years. The Commission has consulted with, and taken account of comments made by, representatives of a number of finance sector associations in changing Forms PQ and PD.
The application form for a personal fiduciary licence has also been modernised and amended to take account of the Rehabilitation of Offenders legislation.

These forms are also available on request from the Commission. The new forms should be used where possible with immediate effect. As institutions or individuals may be in the process of completing the "old" forms, the Commission is content to accept these until 31 July 2006. After that date submissions must be received on the new forms.
The Commission has also undertaken work to seek to ensure that the policies and procedures of the four regulatory Divisions are consistent and, as part of this process, we are making modifications to several of the application forms for other types of licence. The revised forms will be made available on our website in the near future.
